Core Consulting & Service Package

Initial Consultation - Consultation with Leadership Team (1–2 hours)
A focused strategy session with Executive Leadership to assess organizational priorities and initiate key infrastructure development. 

Phase 1: Program Development & Foundation
Timeline: 30 Days

  1. Organizational Needs Assessment (4-hour interview)
  2. Two-Four Day Internal TFH Training for Leadership and Key Stakeholders 
  3. Delivery of Comprehensive Work-Guide Model & Materials
  4. EHR Implementation
  5. Policy & Procedure, Contractual and Implentation Forms Development
  6. Medicaid Billing & Financial Operations Setup
  7. Program Structure for Scalability & Profitability
  8. Identify Staff Leading TFH Through Launch - Recommend Youth Director or above
    Hire Coordinator
  9. Begin Provider Recruitment
  10. Weekly Project Check-Ins with HHC

 

Phase 2: Internal Training & Program Build-Out
Timeline: 90 Days

  1. Weekly Project Check-Ins with HHC
  2. Five-Day Onsite Provider 40 Hour Pre-Service Training Facilitated by HHC
  3. Licensure Prep & Facilitation
  4. Prompt Extension of Provider Contracts
  5. Prompt Extension and Securing MOU Agreements
  6. Compliance & Risk Management Consultation


Phase 3: Program Implementation & Client Onboarding
Timeline: 60 Days

  1. Intake and Staffing of Clients Begins
  2. Implementation of Systems Developed in Phases 1 & 2
  3. Field Testing and Live Adjustment of Tools and Protocols
  4. Monitoring Initial Service Delivery and Fidelity
  5. Audit Preparation Begins
  6. Active Support of Program Implementation and Early Troubleshooting
  7. Weekly Project Check-Ins with HHC

Phase 4: Program Sustainment & Optimization
Timeline: 180 Days

  1. Ongoing Program Checks, Targeted Troubleshooting, and Interactive Fine-Tuning
  2. Support Preparation for Client's First Quarterly DMH Audit
  3.  Progress Tracking Against Implementation Milestones
